HMB, also known as hydroxymethylbutyrate or β-hydroxy β-methylbutyrate, is a compound found in the human body as an active metabolite of the essential amino acid leucine. This compound has been studied for over 25 years in close to 100 clinical studies, including over 20 studies involving healthy or hospitalized adults and other nutrients.

The effects of HMB on skeletal muscle seem to be linked to its influence on both muscle protein synthesis and muscle protein breakdown. HMB helps enhance muscle protein synthesis by stimulating a regulator called mammalian target of rapamycin (mTOR), which plays a role in cell growth and differentiation. Besides, HMB may also boost the activity of the growth hormone (GH)–insulin-like growth factor (IGF) axis.

On the other hand, HMB is thought to decrease protein breakdown by inhibiting the body's ubiquitin-proteasome system and caspases. These pathways are responsible for breaking down proteins during catabolic states, such as fasting, immobilization, and aging. As such, HMB may help minimize the loss of lean body mass in specific situations.

Supplementing with 3 grams of HMB per day has shown to be well tolerated. This is the standard dose used in research studies.
